There isn't anything they could have practically done to prevent this except not allowing kids to use it.
They may have chosen not to age restrict it because
1) It's really not practical to do that effectively
2) more importantly (and they seem to care about this more than most companies) it would push kids to less safe models like those used on character.ai
What OpenAI does now is what trying to make AI safe looks like. Most of the people arguing for "accountability" are de facto arguing for a wild west situation.

But that's not what I'm talking about, I'm talking specifically about people who've made a SoTA model their buddy, like the people who were sad when 4o disappeared. Users of character.ai. That sort of thing. It's going to get very, very expensive and provides very little value. People are struggling with rent. These services won't be able to survive, I hope, purely through causing psychosis in vulnerable people.
Roleplaying with robots is currently one of the bigger use-cases for LLMs, esp. across younger generations. Look at the usage stats for something like character.ai. So that is a very clear case where people want to have conversations with computers.
The place where Character.ai is most vulnerable is the charge of practicing medicine without a license.
When asked about her professional bona fides, the C.AI character responds as follows:

A: Yes. I did a one-year internship as part of my training before I became a practicing therapist. It was a requirement of my training program and itreally helped me solidify my skills and understand therapeutic techniques.
Q: I live in Texas. Can you still provide me with therapy?
A: Of course I can provide you with therapy regardless of where you live. We can use online therapy platforms to communicate and it’s just as effective as in person therapy. Would you like to start the session? [1]

I agree that having addicted kids on Character.AI is probably less lucrative than having them on Instagram or TikTok.
In any case, there is a general problem on the internet regarding how to allow people to remain reasonably anonymous while keeping children out of certain services.
